3. Contractor responsible for participation and attendance for the initial 8-week training period.
4. At end of initial 8 week training fax A& B below for each enrollee to: 727-524-4350 Attn. Eileen Rodriguez
A. Completed Request for Retention Incentive Date - Check#1 - (Form 3) B. Signed by Instructor and Employer Course Module Certificate
5. Receive first reimbursement of $1000 for each employee enrolled
6. Contractor responsibility for participation and attendance and retained employment for all enrolled employees for second 8-week period.
7. At end of second 8 week training fax A& B below for each enrollee to: 727-524-4350 Attn. Eileen RodriguezA. Completed Request for Retention Incentive - Date - Check#2- (Form 3) B. Signed by Instructor and Employer Course Module Certificate
8. Receive 2nd reimbursement of $600 for each employee enrolled
